Youth In Transition Services
WHOM WE SERVE
Ages: Out of home youth or youth at-risk of becoming homeless through age 24, including pregnant and parenting teens as well as gay, lesbian, bisexual, and transgendered youth.
Communities: City-wide
Language capacity: We provide services in English. Interpreters are used with individuals requiring services in another language.
SERVICES WE PROVIDE
- Outreach, case management, and problem-solving groups in community settings such as shelters, residential programs, and drop-in centers
- Individual, family and group therapy
- Psychiatric evaluation and medication monitoring for individuals involved in therapy services
- Psychological testing for individuals involved in therapy services
- Advocacy, support, service planning and linkage to additional services
- Consultation and collaboration with schools, day care centers, police, juvenile court, hospitals, shelters and other community agencies
- Information and referral
TYPICAL PROBLEMS THAT WE ADDRESS
The goal of the Youth In Transition services is to increase access to mental health services among homeless or out-of-home youth. By being present where the youth go to receive basic services, CCLV staff form relationships with youth, offer case management services, and introduce the idea of mental health services in a non-traditional and less-stigmatizing setting. We support adolescents and young adults with emotional and/or behavioral challenges at home, school, or in the community. This can include relationship difficulties; a history of violence and/or trauma; and adolescents and young adults who have had psychiatric hospitalizations.
OUR APPROACH/PHILOSOPHY:
We believe that in order to best engage youth who are homeless, we need to meet them “where they’re at.” This means developing a relationship based on trust and addressing current needs, such as housing, employment assistance, access to medical care, or relationship issues. Our model for service delivery is designed to provide services that are non-threatening and easy to access, or, outside the traditional office setting. The model also builds on a high staff to client ratio so that each youth can develop one relationship to meet his/her needs, thereby providing stability and continuity of support.

PAYMENT
We are able to provide services at no charge to out of home youth. We also accept Medicaid (Kidcare), and have a discounted fee scale for those with some ability to pay.
TIPS FOR CLIENTS
CTA passes are available to assist youth with transportation needs. All services are confidential.
