OUTPATIENT PROGRAM - ADULT MENTAL HEALTH

Ages: Persons 18 and above
Population: Persons facing severe and persistent mental illness.

Language capacity: English-speaking staff only; professionally-trained interpreters are used for persons in need of language assistance, including ASL.

- Schizophrenia
- Bipolar disorder
- Severe depression

Public transportation: Our office is around the corner from the Belmont station,
on the Red, Brown and Purple lines.
PAYMENT:
We are able to bill Medicaid (with no co-pay), Medicare (which requires a co-pay), and private insurance (generally with a co-pay) for certain services. For persons living in the Lake View area who do not have insurance, we are able to subsidize care through a grant from the Illinois Department of Human Services Division of Mental Health. The co-payment will be determined based upon household size and income.
TIPS FOR CLIENTS:
When calling for services, you will most likely need to leave a number at which we can call you back. Every effort is made to return calls before the end of that workday. However, we may not be able to return calls received toward the end of the day until the following day. Similarly, there may be a delay of one day when we experience an unusually high number of requests for services. An Intake Worker will do a phone screening which typically takes 20 minutes. If we are the right place for you, we will then schedule you for an intake appointment. If we do not provide the services you need, we will provide you with referrals.
It is best to arrive early for your initial appointment so you have time to complete the paperwork required to begin treatment. Some of the paperwork can be completed prior to meeting with the Intake Worker and some is completed during the first meeting.
Everyone is asked to have a complete physical exam early on in treatment unless one has been done within the last year. If you do not have a primary care physician, we are able to refer you to a medical care professional whose fees are matched to meet your financial resources.
IMPORTANT: If you are unable to make your appointments, please call within 24 hours so the time can be used to serve other clients.
Because physical and emotional wellness are interconnected, we ask that you bring with you current medications or pharmacy labels showing all medications currently being prescribed to you.
If you have Medicaid and/or Medicare benefits, please bring your card(s) to the intake. We will make a copy for our records.
