Adult Intensive Case Management Services (PATH and Sojourners)


WHOM WE SERVE: 

Age:                               22 years and older

Population:                    Persons with Severe Mental Illness; those with a history of
                                       inpatient  psychiatric hospitalizations  

Communities:               North lakefront east of Chicago River

Language capacity:     At this time, staff limited to English language; interpreters will be
                                       utilized as necessary.

 

SERVICES WE PROVIDE:

  • Crisis intervention and stabilization.
  • Assistance in securing and maintaining housing and food.
  • Financial assistance including representative payee and linkage to entitlements.
  • Advocacy, referral to community resources and linkage to a continuum of mental health and health services. including outpatient services, psychiatric care and medication monitoring, substance abuse treatment, vocational rehabilitation, etc.   
  • Supportive counseling and problem solving in developing community living skills.
  • Individual and group rehabilitation stabilization services that assist the individual in improving and/or maintaining the ability to function appropriately in a variety of interpersonal situations.

TYPICAL PROBLEMS THAT WE ADDRESS

  • Homelessness
  • Lack of income to support oneself
  • Inability to manage one’s money appropriately
  • Lack of family and/or social supports
  • Need for assistance with medical and psychiatric appointments and medication regimes.   

 

OUR APPROACH / PHILOSOPHY:

Both adult case management programs work to prevent psychiatric hospitalizations where possible and appropriate and to reduce length of stay for individuals who are re-hospitalized.  We strive to improve the quality of life for severely and persistently mentally ill individuals by offering assistance with stabilization in the community.

 

REFERRALS:

Most referrals come from other either state or private hospitals, social service agencies, other case management programs due to the short term nature of their programs, and occasionally private therapists and probation officers. 

 

WHERE WE ARE:

Services of Sojourners and PATH are provided in the community, at the client’s residence or other locations

 

PAYMENT:

Most individuals eligible for case management services are eligible for Medicaid.  Most individuals are homeless at the time of entry into the program and would not be charged a fee for services.  Individuals with an ability to pay would be charged at the agency case management rate or in accordance with the subsidized fee schedule

OTHER FACTS YOU SHOULD KNOW:

Sojourners and PATH are community based programs.  Psychiatric appointments, individual therapy, and group services are provided at the Counseling Center of Lake View.
