Older Adults Home-Based Mental Health Services (Older Adults Program)

OLDER ADULTS PROGRAM

 

WHOM WE SERVE 

Ages:

Adults 55 years of age and older

Communities:

Lake View and the contiguous north side communities of Lincoln Park, Ravenswood, Uptown, Edgewater, Lincoln Square and North Center

 

TYPICAL PROBLEMS WE ADDRESS

          Individuals

  • Troubled by psychological, emotional, cognitive and/or environmental problems
  • Experiencing difficulty adjusting to issues of aging such as loss, physical changes, isolation
  • In need of financial management, making a decision regarding where to live,  planning for the future, etc.
  • Who have had psychiatric hospitalizations

SERVICES WE PROVIDE

  • Individual counseling (often in a person’s home)
  • Family counseling
  • Psychiatric consultation, evaluation and treatment
  • Case management including money management
  • Assistance with home tasks
  • Linkage, collaboration and advocacy for other needed services

WHO PROVIDES THE SERVICES

  • Masters and bachelors prepared human service professionals with expertise in mental health and aging
  • Psychiatric consultants
  • Highly qualified workers with specialized geriatric experience

OUR APPROACH  /  PHILOSOPHY

We are a low key, flexible program that meets the older person where they are at.   Individuals present their problems in different ways and we accept their own description of their needs.  We recognize the vulnerability of seniors with mental health difficulties and the chaos it can bring to their lives.  We seek to work with them to achieve the highest quality of life possible.

 

FUNDING AND PAYMENT

The community arm of the Older Adults Program is partially funded with grant money from the Division of Mental Health and the United Way.   We are able to bill Medicaid (with no co-pay), Medicare (which requires a co-pay), and private insurance (generally with a co-pay) for certain services.  If applicable, we have an adjusted fee scale for those with limited income.
