PSYCHOSOCIAL REHABILITATION DAY PROGRAMMING
WHOM WE SERVE:
- Adults 55 years and older who live on the north side of Chicago or near north suburbs
- Isolated older adults who may have had a recent hospitalization and need the structure, continuity, and organization of a day program
- Seniors troubled by psychological, emotional, and/or cognitive problems and would like to:
- Make new friends
- Learn new skills
- Participate in fun-filled activities
- Create, discover and grow
SERVICES WE PROVIDE:
At 21st Century Seniors, we provide a five hour day from 10:00 AM to 3:00 PM, Monday and Tuesday and a five and one half hour day from 10:00 AM to 3:30 PM, Wednesday through Friday. Each day is filled with learning, creative activities, outings and community building. We serve a hot, nutritious lunch-time meal and two snacks daily. Transportation is addressed in three ways:
- Bus passes are provided for those able to travel unassisted
- Pick-ups are offered in our van for those within our catchment
- Arrangements are made for those living outside of our boundaries
WHO PROVIDES THE SERVICES:
- Masters and bachelors prepared human service professionals with group expertise
- Art therapist
- Masters and bachelors prepared nurses
- Psychiatric consultants
- Auxiliary personnel sensitive to the needs of seniors
OUR APPROACH / PHILOSOPHY:
21st Century Seniors is a wholistic model of care that draws on the principles of wellness, prevention and strength to address seniors' problems. The program uses an upbeat, growth-oriented focus that contradicts the traditional perspective of aging, particularly of aging adults with mental illness. We use a variety of strategies such as group games, arts, and psycho-education. The significance of the program is in its values: empowerment, self-sufficiency, wellness, hope, meaning, possibilities for growth and, relationship and community building. These positive elements influence the tenor, mood, and atmosphere of the program.

21st Century Seniors is located at 1908 W. Montrose Avenue in Chicago. We are one block west of the Ravenswood L stop and on the Montrose bus route.

FUNDING AND PAYMENT:
The Psychosocial Rehabilitation Day Program receives limited funding from the Office of Mental Health, the United Way, and the Department on Aging. We are able to bill Medicaid. If applicable, we have an adjusted fee scale for those with limited income.
